„Reversibel" ist eines der stärksten Argumente für phake Linsen – aber was bedeutet das konkret?
Eine phake Linse kann über einen kleinen Zugang wieder entfernt oder gegen ein anderes Modell bzw. eine andere Größe getauscht werden. Typische Gründe sind ein dauerhaft ungünstiger Vault, eine relevante Refraktionsänderung über die Jahre oder – selten – nicht tolerierte optische Phänomene. Wichtig bleibt die ehrliche Einordnung: Entfernbar heißt nicht risikofrei rückholbar. Jeder Wechsel ist erneut ein intraokularer Eingriff.
Irgendwann trübt sich jede natürliche Linse – mit oder ohne Implantat. Entwickelt ein Patient mit phaker Linse einen behandlungsbedürftigen Grauen Star, wird in derselben Operation die phake Linse entfernt und die getrübte natürliche Linse durch eine Kunstlinse ersetzt. Die phake Linse verbaut also keinen späteren Behandlungsweg – im Gegenteil: Bei der Kataraktoperation stehen dann alle modernen Linsenoptionen offen, von monofokal bis Full-Range-of-Vision.
Die Kataraktentwicklung selbst ist mit modernen Aquaport-Designs und korrektem Vault deutlich seltener geworden – bleibt aber der wichtigste Grund für die lebenslangen Jahreskontrollen.

A phakic lens can be removed through a small incision or exchanged for a different model or size. Typical reasons are a persistently unfavourable vault, a relevant refractive change over the years, or – rarely – optical phenomena the patient cannot tolerate. The honest framing remains: removable does not mean risk-free retrievable. Every exchange is once again an intraocular procedure.
Every natural lens eventually becomes cloudy – with or without an implant. If a patient with a phakic lens develops a cataract requiring treatment, the phakic lens is removed in the same operation and the cloudy natural lens is replaced with an artificial one. The phakic lens therefore does not block any later treatment pathway – on the contrary: at cataract surgery, all modern lens options remain open, from monofocal to full-range-of-vision.
Cataract formation itself has become considerably rarer with modern Aquaport designs and a correct vault – but it remains the most important reason for lifelong annual check-ups.
